SmartSense PPO

These are our most affordable plans offering basic coverage. A choice of six plans with six different deductibles lets you choose your level of savings. And no matter which plan you choose, each member will have coverage for his/her first three in-network doctor visits each year -- before you have to meet your plan’s deductible. You’ll also have coverage for generic drugs – with optional coverage available for brand name and specialty drugs.

Premier PPO

If you have young children -- or are planning to have a family – you’ll find that Premier PPO health plans were created with your specific needs in mind. You’ll enjoy copays that are designed to be affordable for doctor visits, routine exams and other preventive care services, like an annual vision exam, as well as coverage for prescription drugs. Customize your ideal plan by choosing from six deductibles and adding optional maternity benefits.

BlueChoice High Deductible Health Plans

High Deductible Health Plans (HDHPs) that meet certain requirements established by the U.S. Treasury Department are designed to work with tax-advantaged Health Savings Accounts (HSAs) to provide customers with flexibility and choice, along with financial incentives to become more involved health care consumers.

Short-Term PPO

Change is a part of life. And sometimes it can affect your health benefits. Like when you’re starting a new job and your health coverage hasn’t kicked in yet. For times like these, a Short-Term PPO plan can help fill the gap. You even have a choice of payment options! Just keep in mind that, because this policy offers coverage for a limited period of time, any illness, injury, or other condition you have or have had within the past 24 months may not be covered. Ask your sales representative for details.
